How do I create a Plan Form Template?

A Plan Form is a questionnaire created by an administrator for travelers to fill out when added to a Solo or Group Travel Plan. Administrators can use a combination of Form Components and Form Questions to customize a Plan Form Template to collect critical pre-departure information from travelers. When a Plan Form is published it can be manually or automatically added to Travel Plans using configurations in Organization Settings within the Via Travel tab. To create a Plan Form...

  1. Click Builder in the main navigation menu
  2. Click Create New
  3. Select Form Type Plan Form
  4. Click Next
  5. Type in an Plan Form Name
  6. Click Create

All Plan Forms default to having First Name, Last Name and email address on the form. Via recommends keeping these data associated with each Plan Form for easy searching, sorting and reporting!
